Dr. Vasant Lad

Ayurvedic approach to Asthma

In ancient Ayurvedic literature Asthma comes under Swasa Roga, one oft he major ailment of Pranavaha Sroto Dushti. Swasa means undue awareness of respiratory distress. It could be a symptom of some other major diseases like congestive heart failure, pleuricy with effusion, obesity, ascites and uraemia. One has to find out wether this present symptom of breatlessness is truely of Asthma or other condition because the entire treatment protocoll depends up upon on this differential diagnosis. In bronchial asthma the air is drapped in the alveoli therefore there is difficulty of exhalation. Inhalation is relatively easy but exhalation is prolonged and labourious. Acharya Charak while describing the ideological factors of Pranavaha Sroto Dushti says Dhumopaghat (exposure to smoke or chemical fumes), Rajopaghat (exposure to dust), Vyayam (vigourous exercise), Rukshaanna the dry food  and Vimargagaman (false entry of food into the trachea) can create respiratory tract disorder. And you will be surprised that these factors are still present in the modern era.

Nidan (Etiology)  of Asthma

¨ Beeja Dosha (heriditary) genetic predisposition

¨ Asatmya (allergies)

¨ Sheetaseva (exposure to cold damp wether)

¨ Rajopaghat (exposure to dirt, dust and debree)

¨ Dhumopaghat (exposure to smoke, chemical fumes)

¨ Ojo Dushti (immune dysfunction)

¨ Mano Vikruti (Stress)

¨ Kaphagenic food (diary, cheese, yoghurt, icecream)

¨ Krumi (paracites, wormes)

Samprapti (pathogenesis) of Asthma

Due to above mentioned kaphagenic factors the Kledaka Kapha undergoes sanchaya (accumulation), prakopa (provocation) in the stomache and prasara (spread) in the Rasa dhatu. Via media of Rasa dhatu (serum) the Kapha goes into the lungs creating increased bronchial secretion causing congestion and air gets drapped in the alveoli by producing Asthmatic attack. This is the book picture of Kapha type of asthma which begins in the stomache. And it needs specific seperate herbal protocoll.

If the etiological factors are Vata provoking like dry foods, overexercise and dry wether can cause aggrevation of Vata in the Colon which goes into the lungs via Rasa dhatu. Vata is dry and cold in nature can produce dry caugh and narrowing of the bronchial tree, thus air get drapped in the alveoli producing dry wheezing. This inbrief is the picture of Vata type of Asthma which also needs a seperate protocoll.

Due to Pitta provoking causes such as hot, spicy food and irritating smoke, summer season the Pachak Pitta from the small intestine (Grahani) undergoes accumulation, provokation and spreads into the Rasavaha srotas. This Pitta finally lodges into the bronchial tube causing inflammation, irritation and infection of the bronchi. Due to inflammatory changes and infection the air again gets drapped into the alveoli and produces difficulties of breathing, cough and irritation of bronchial tube with greenish-yellowish sputum and feverishness. This is a brief picture of asthmatic bronchitis a classical Pitta type of asthma for which Ayurveda suggests a different herbal protocoll.

Even paracytes can produce repeated asthmatic in children which should never be ignored while dealing with asthma.

Chiktisa (treatment protocoll)

The practioner should examine the patient of Asthma sorrowly with Darshana (observation), Sparshana (tactile perception) and Prashna (questioning of past history, history of previous illness and family history). Then one should do thorougho clinical assessment of the client and determine the Prakruti-Vikruti paradigm in order to detect the type of asthma and then do Dosha pratyanika that is specific to Dosha, then Dhatu pratyanika specific to Dhatu and ubhaya pratyanika specific to both and create a specific individualistic herbal protocoll. In the above modell of asthmatic patient who is strong with udklishta Dosha (profused lung secretion) give Vaman without Purvakarma, that means dirctly Yasthimadhu and Madan Phalla Churna together to induce vomiting that will immediately remove the excess Kapha from the lungs and give the patient freedom of breathing.

On the other hand if you find insufficient secretion of lungs in the strong patient then  we do Purvakarma that is Pre-Panchakarma preparation and then induce Vaman which will also help to exite Kapha and eliminate it out giving freedom of breathing tot he patient.

After Vaman we have to do specific herbal protocall for Kapha type of Asthma. The second category of weak patient with profused lung secretion they cannot tolerate Panchakarma hence we give expectoration therapy such as Sitopaladi, Yastimadhu, Purnanava, Abhrak and Pippali with honey three times a day.

The next category of the weak patient is insufficient lung secretion where no Panchakarma is recommended but Shamana and Rasayana Chikitsa is advisible. Following or the specific herbal protokall according to Doshic imbalance in the patient of Asthma:

Kapha type

¨ Purnanava, Sitopaladi, Yasthimadhu, Abhrak Bhasma

¨ This formular together ˝ TSP three times a day with honey

¨ Purnanavasava, Vasakasava, Pippaliasava 4 TSP with equal water after lunch and dinner

¨ Give Bibitaki virechana 1 teaspoon at night with warm water.

Pitta type

¨ Shatavari, Sitopaladi, Mahasudarshan, Moti Bhasma and Swans Kuthar. This formular ˝ TSP three times a day with dadimavaleha (Pomegranate syrup)  or Aloe vera gel 2 table spoons three times a day.

¨ Give Amalaki virechana 1 tsp at night with warm water.

Vata type

¨ Dashmoola, Ashwagandha, Talisadi, Abhrak Bhasma and Pippali. This mixture ˝ TSP three times a day with vasavaleha.

¨ At night give Haritaki 1 TSP at night with warm water.

Protokall during acute attack

We have no time to see wether it is Vata, Pitta or Kapha Asthma, but following formular works instantanously:

Liquorice 1 TSP with one cup of water boiled together for 2 min., then add 5-10 drops of Mahanarayan oil, mix it sorrowly and give one sip every 10 min. and rub Mahanarayan oil to the chest and apply bearable heat to the chest wall by hot water bag than you can say Asino labhate saukhyam (patient of Asthma feels better in sitting posture) that’s why he is proped up with pillow and graving for hot drink. So this liquorice herbal protocall relieves congestion, does bronchio dilatation and patient feels instantaniously better. This treatment works faster than any nebulizer.

Rasayana for Asthma

For Vata type: Dashmoolarishta and Pippali Asava, Pippali Sidha milk

For Pitta type: Vasakasava and Somasava

For Kapha type: Vardhanmana, Pippali Rasayana, Pippali Avaleha, Chyawanprash

In patient of Asthma there are ancient specific Marmani (energy points) on the surface of the body by pressing gently but deeply at these energy points which opens energy pathways to the respective organ in order to perform their normal function. The following diagramm with specific names oft he energy points can be used in the patient of Asthma to relieve the drapped air in the alveoli. These marmani are the pathways of inner pharmacy. They unfold inner healing power.
